Forms for Scout Use

Patrol Meal Planner - Must be approved by SM or designee prior to food purchase.

Patrol Duty Roster - Must be approved by SM or designee prior to campout.

Troop Election Packet - Print last page and turn in 2 weeks prior to troop elections.

National Honor Patrol Worksheet - To be completed by the Patrol Leader and turned into the Advancement Chair.

Scout Service Hour Record - Use this form to record your service hours for your rank.

Hike Trek form - for the hiking merit badge

7R - Merit Badge Form - Scouts need to turn this in for their selected merit badges for 7R

Forms for Parent Use

Motor Vehicle Registation - you must register your vehicles to be permitted to have scouts in your vehicle.

Health Form - Valid for 1 year

Elks Financial Aid Form - Aid for ANY scouting related expense.

Medication Release Form - This need to be filled out for the Troop leaders, prior to them dispensing any medication.
